To request a Mental Health Evaluation or an Independent Mental Health and Medical Psychiatric Evaluation, case managers, adjustors, or attorneys can initiate the process by calling our Lead Receptionist at 515-635-0213. Our team will collect essential information and guide you through the next steps. We will send a fee schedule and request the case file.

Payment Information:
To streamline our services, we provide a comprehensive fee schedule outlining the costs associated with our evaluations. Upon request, we will promptly send you a W9 form. Prepayment is required to confirm the scheduled evaluation.
